While I believe that from a diagnostic point of view there are significant differences if a full medical anamnesis and diagnosis has been done, the terms you have listed do not reflect such diagnosis. What in my view is missing is a category of people who believe that differentiation for the purpose of appropriate health care is unavoidable.
